Prior to your first appointment please read and sign the documents below - and then email them to me.   I will try to communicate with you prior to the session if I notice something missing or needing your attention.  Ideally, we can complete the administrative work prior to the session so we can get started with the clinical work.

• Scan or photograph 2-5.

• Scan or photograph the front & back of your insurance card.
